What do you think about the Academy proposal?

Please download and complete the questionnaire to share your views on the proposal.

To download the form click on the 'download questionnaire' button below. A box will appear and you should click the 'open' button. A word document will open which contains the questionnaire. Please fill it out completely, it should only take a couple of minutes. You should then save the questionnaire.

Once the questionnaire has been completed and saved you can use the box below to send it to us. Click on 'Browse...' and a box will open. Select your saved file and then click on the 'open' button. This page will appear again and you can then click on 'Send'.
